This very short-lived (in his own words) "cute, sexy and lick-the-mirror handsome" Tenth Doctor came into being when his previous self regenerated after the Daleks "exterminated" him on the planet Tersurus. He "died" when trying to fix the Daleks' broken zegtronic beam and turned in his next self. (DW: The Curse of Fatal Death)
