Ashildr used the spare [[medical kit|immortality chip]] the Doctor had given her centuries before to cancel out Sam's death — the portal was closed and Sam was saved.

"Sam Swift the Quick" was the alias used by a highwayman in the 17th century and the Knightmare's main rival. Like her, he later shared in her fate of an augmented lifespan.
