A spaceman, known as Jack, was part of the Marine Space Corps expedition to Exxilon in search of the rare mineral parrinium, which was found in vast quantities on that world and desperately needed to cure a galactic plague.
While investigating the surface of the planet one night, shortly after the MSC spaceship was forced down by the power drain caused by the glowing beacon atop the Great City of the Exxilons, he was killed by an Exxilon arrow and tumbled down a slope straight into a lake.
When he failed to return, two of his colleagues, Captain Richard Railton and Lieutenant Dan Galloway, went out the next morning to try to find him but, on returning to their base with the Third Doctor, had to tell Jill Tarrant that their search had been unsuccessful. (TV: Death to the Daleks)
