By all accounts, Romanadvoratrelundar’s third incarnation was the one who followed Romana II’s regeneration.
Purged.

Where accounts differ, however, are the particulars surrounding this regeneration. By most accounts, the third Romana served as President of the High Council during an all-encompassing time war between Gallifrey and another major temporal power. (PROSE: The Shadows of Avalon (novel); AUDIO: Luna Romana (audio story)) Other accounts, however, suggest that Romana had eschewed the Presidency during this incarnation and served the universe in other ways. (STAGE: Time Fracture (stage play); PROSE: The Little Book of Fate (short story))

According to one account, Romana II regenerated into a snub-nosed woman with dark hair styled into a fringe who apparently resembled the Doctor's mother at some point during the first decades of her Presidency. (PROSE: The Shadows of Avalon (novel))

By this account, Romana would lead Gallifrey as it approached an all-encompassing temporal conflict with the Enemy known as the War in Heaven. (PROSE: The Ancestor Cell (novel), et al.) However, after Faction Paradox invaded Gallifrey, the War was averted and Gallifrey was destroyed. This resulted in a new form of reality where history was “fundamentally altered” and both Gallifrey and the Time Lords were wiped from the timeline. (PROSE: The Ancestor Cell (novel), The Adventuress of Henrietta Street (novel))

She would go by title such as the War Queen and Mistress of the Nine Gallifreys.

In an timeline which was later undone, Romana II regenerated in order to prevent the Moros from becoming a black hole and destroying Gallifrey. (AUDIO: Enemy Lines (audio story)) This version of Romana III led Gallifrey throught its "darkest hour", (AUDIO: Luna Romana) including attempting to thwart the Adherents of Ohm. (AUDIO: Intervention Earth) Nevertheless, Omega did return to Gallifrey war devestated it. (AUDIO: Enemy Lines (audio story))

While she ultimately orchestrated her own timeline's undoing through Irving Braxiatel, a Matrix projection aided her previous self in numerous encounters (AUDIO: Renaissance (audio story), Ascension (Gallifrey audio story)) leading up to the Last Great Time War. This incarnation worked with Laszlo and other Tharils to rescue refugees from a "forever war spreading through all of time and space." This temporal refugee camp was disguised as a traveling carnival, in which she posed as a fortune teller. She was similar to the Romana III who led Gallifrey through its darkest hour by the fact they both wore a black beret. (PROSE: The Little Book of Fate)

Within a temporal anomaly known as the Time Fracture, time began to repeat over itself. At some point during this time loop, Romana II regenerated into a non-binary incarnation with short, blonde hair and blue eyes. No longer holding the Presidency, they instead held the title of Lord Cardinal. While UNIT had speculated that they would make for a powerful ally should they survive Gallifrey's destruction, a flood of artron energy closed the Time Fracture and prevented it from ever having opened.source

Behind the scenes
* Writers involved in the War in Heaven arc of Eighth Doctor Adventures novels based Romana III's described appearance on that of the American silent film actress Louise Brooks.

* James Goss contrasted Juliet Landau's version of the third incarnation from "other future Romanadvoratrelundars" like the "brilliantly glacial flapper" who had appeared in the Eighth Doctor Adventures. (VOR 56) Despite this, Landau bares a striking resemblance to her prose counterpart.
