The First Doctor used a holographic visa was when he visited Bukol with Steven Taylor and Dodo Chaplet. It was shaped like a light bulb.

When the Doctor passed through customs, the image on the visa did not match his appearance. Unknown to him, Mykloz had switched the Doctor's visa with another person's (who was actually the Sixth Doctor). The Doctor had to find a way to prove his identity to Bukolian official Yerma.
