"Doctor Who and" is a common phrase that begins a multitude of Doctor Who story titles, mostly those from the Target novelisations range. However, it was the usual in-house pre-cursor on televised scripts. The process of titling scripts "Doctor Who and ..." was abandoned by Barry Letts, after the BBC graphics department accidentally tacked on the phrase to create Doctor Who and the Silurians. Because the title was self-referential, and it suggested that the Doctor's name actually was "Doctor Who", scripts thereafter excluded the phrase.
