According to Director Stahlmann of the Inferno Project, he was killed in a car accident while on his way back to London from Eastchester the day before the Doctor's arrival in his universe. (TV: Inferno)
Behind the scenes
- As Great Britain on Inferno Earth had been a republic for 31 years, the Royal Family having been executed, it is unclear how Sir Keith came to have his title as he could not have been knighted. However, it is possible that aristocratic titles continue to be part of family names in the British Republic while having no legal recognition, as in real world Germany.
