Not really a production error?
I do believe that under 'production errors', this bit "In part five, Rex Robinson tries to say, "We drove the Ice Warriors out of the mines but we couldn't break through to the Citadel," but stumbles on the word, "warriors"." is simply untrue.
I've just watched episode 5 and played that one line over and over again to hear him "stumble" but no dice, it sounds completely correct. Why is this here?
