The statement that "An unspecified incarnation of the Doctor was revealed in a twist ending to have been silently watching the entire video call" is an assumption. It isn't actually revealed. Yes, it's the assumption that the story is leading you towards, but it isn't explicit. There is room for manoeuvre. Who knows? - The use of the Doctor's theme on the soundtrack rather suggests that it's the Doctor. In terms of basic literacy from a RW perspective, I think it's clear that the Doctor is featured in this story.
