Please:
Note that admin are only required to upload a video. Anyone can put the video on the page once it is uploaded by an admin. For reasons of efficiency, therefore, admin may choose to simply upload, but not put it on the page. Admin will leave behind a note clearly stating whether they've actually put it on the page.
Remember, we only accept videos from the YouTube accounts of copyright holders, as outlined in our video policy. Here's an example of the form, but do please make sure you read the instructions for the {{vrec}} template:
