To be fair, it's never been said that it's the first time the question is asked that that's when Silence will fall. When we were told "he will knock four times" we didn't believe that the first time that someone knocked four times that the Doctor would die. Rather, we believed that when the Doctor died would be when someone would knock four times. In fact, given it's supposed to be the oldest question in the universe, it's pretty much guaranteed that it would have been asked before. So "Who is River Song?" is still perfectly valid.
