The Dream Lord was the dark side of the Doctor, who seems to have control over dreams. He was created by psychic pollen working as parasites on the Doctor's inner darkness. Despite the Doctor having confronted him, as he looked at his reflection, the Dream Lords face appeared, laughing maniacally at him.
