As explained in the ending, it was all very simple: The Time Lord known as The Doctor has had 12 bodies, 11 of them were known as 'The Doctor' save one, the one played by John Hurt. The reason why the John Hurt Doctor isn't called 'The Doctor' is because he did something so horrible, he could in no way ever be considered 'The Doctor', the word for healer or wise man. Hopes, wishes, and random speculation aside, that is what the facts and hints point to. However, I'm wondering if Hurt might actually be Doctor 8.5. Didn't see 2-3 regeneration and the ''Doctor'' ended the Last Great Time War. Hurt isn't the Doctor. Although DWI 12 said he had rejected that name, so the Eighth Doctor could have commited suicide in the name of the Doctor, to end the Last Great Time War, because he knew that the Hurt Doctor (the Storm?) would activate the moment, so he wouldn't disgrace the name of the Doctor, thus cutting off that incarnation.
