Bartholomew Kitchen was a BRigadier-General in the Crimean War. He was a bit mad, due to that fact that he had betrayed his country, in an attempt to finish the war. He blamed The Doctor for it. He believed that he killed the Doctor, but he actually disappeared in a white box. A month later, he resaw that box after he shot Hex. (BFA: The Angel of Scutari)
