Mike Yates
The announcements and media coverage for Hornets' Nest all describe Yates as the Fourth Doctor's companion, so I think adding him to the list is justified. 23skidoo 05:23, 3 August 2009 (UTC)
K9 Mark III
Prior to switchover to new format, template maintained that K9 Mark III was a prose companion of the Fourth Doctor. What's the source of this?

Potential Additions
Milena from Milena is overtly reffered to as a companion in her story, despite her brief tenure. Meanwhile, Miss Young from The Sea Devil also only has one appearance, but clearly serves the role of the companion -- with clear indication that the two have been having adventures for some time and that afterwards that it's back to the TARDIS! Companion's from The Drosten's Curse
The Fourth Doctor novel The Drosten's Curse features two characters who serve as companions: Bryony Mailer (whose page was recently created and reminded me of this) and Putta Pattershaun 5 (who as of this writing does not have a page, though I can create one).
