David Forman, also credited as Dave Forman, was a stunt co-ordinator for several episodes of BBC Wales-produced Doctor Who. These episodes included Tooth and Claw, The Girl in the Fireplace, The Idiot's Lantern, Fear Her and The Runaway Bride.
