It's capitalised in the novelisation, Divided Loyalties, The Nightmare Fair and its audio adaptation (which had The Celestial Toymaker on the cover)... If it was ever a mistake, it's not anymore

He was called the "celestial Toymaker", with celestial being just an adjective, not part of his name. In the same way you might say "the amazing Doctor". But then writers assumed it was part of his name. We really need to expand our behind the scenes coverage of the Toymaker.

Yes, yes it very much is an error. He was always scripted as "the Toymaker", with "Celestial Toymaker" always being intended to be in the sense of "The Amazing Doctor", or something, not part of the character's name. It's only the writers in the 70s, 80s, 90s, and 00s who missed the trick and got it flat out wrong.
