: I'm not sure about the first two, but with regard to your third point, it's not traveling through a wormhole unprotected that kills people; it's traveling through the time vortex. Granted, there was the wormhole in "Planet Of The Dead" that disintegrated anyone who crossed it, but wormholes may not all be identical to each other. Plus, the TARDIS may have projected a "bubble" around itself to keep the Doctor safe, like how it creates bubbles of air when the doors are open in space. I assume that it would not have done the same for Jack since it wanted him gone.
