It makes a refer-back to the Original Doctor, who the Eleventh claims visited Akhaten with Susan.
This is one of the few times that the Doctor is shown in a negative light, as he forces Clara to give up items of great sentimental value to her. It's also another time where the Doctor is forced to relive some of his worst memories, as he notes several things he's seen.

The ''Impossible Girl'' arc continued in this episode, with the Doctor not being able to find any answers as to why there are multiple Claras throughout history. So far, it appears to have been nothing more than a coincidence.
