There was one in Amy's Choice too - it's outside in space, but is visible on the TARDIS monitor when the Doctor identifies the Cold Star. But it's very small, and at an angle, so you need to look carefully. There was one in the Vampires of Venice too - in the storm clouds at 42 mins 33 secs.
