Considering the varying degrees of metafiction in Dicky Howett's other work, it is never made explicit in this story that the Doctor is not a real person. As Howett stated in his autobiography, Drawing Breath, Movie Millie was an ongoing feature within the pages of Club International. He also elaborated on the Doctor's identity, saying that Millie was "a porn star who, every issue, was teamed with Big John, a prodigious chap who could extend himself at least forty-five feet".
