Titania was a space shuttle operated by the British Space Programme in the 1990s.
After a mission to the dark side of the Moon, Titania was destroyed whilst re-entering Earth's atmosphere. Although investigations by UNIT and Torchwood found no evidence of anything other than an engineering flaw being responsible for the disaster, the shuttle had actually been targeted by aliens, whose attempt to kill the crew resulted in astronaut Helena McNamara's mind being separated from her body.
