A Chameleon took over the identity of Blade.

"Blade" became the chief pilot of Chameleon Tours and controller of their operations.
When the Second Doctor defeated the Chameleons, the false Blade was willing to negotiate to save his people. When the Director attacked him in an attempt to regain control, "Blade" was forced to shoot and kill him. The real Blade was subsequently freed. (TV: The Faceless Ones)
