The Fifth Doctor regenerated into a sixth incarnation after trapping himself in his TARDIS with the Smoke until all of his lives were up. Abby and Zara were able to use their powers, however, to restore him in his fifth incarnation after the Smoke died due to having nobody to feed on. (AUDIO: The People Made of Smoke)
