There is no hard rule, but it seems like renegade Time Lords, like the Doctor, the Master, the Monk, and the Rani, are the ones who adopt titles. For example, in The Dimension Riders, the Time Lord Epsilon Delta goes renegade and decides to adopt the title "the President".
