If we look at that episode and consider the Doctor's relationship with Bill and The Captain, I could see them qualifying on the same basis as Bret Vyon under "narratively ambiguous" and propose they be added to the list on that basis.

Well, I've taken away the "Original to..." sections to avoid reptation of groups and lists, then merged them based on the type of companion they are; those that travelled for a longer time as oppose to those with that were on the TARDIS for a series of consecutive trips that made up one adventure.
