If we look at that episode and consider the Doctor's relationship with Bill and The Captain, I could see them qualifying on the same basis as Bret Vyon under "narratively ambiguous" and propose they be added to the list on that basis.

- Well, I've taken away the "Original to..." sections to avoid reptation of groups and lists, then merged them based on the type of companion they are; those that travelled for a longer time as oppose to those with that were on the TARDIS for a series of consecutive trips that made up one adventure.
