This is partly to keep someone new from changing an image that the community already changed. Popular pages include, but may not be limited to, the Doctor, the Master, any companions, and television episodes. The wording on the policy page may not be all that clear; if you like I can amend it to be more clear.
And it's not so strange that the original image was never voted on; it was either placed before the rule was codified, or it got in under the radar. These things happen on a large wiki.
