Secondly - Doctor Who unambiguously exists in-universe. It has been a fact since comic stories from the 1960s. There are literlly dozens of sources that corroborates its existence.

Thirdly - there is no evidence within Doctor Who and the Fangs of Time to suggest it takes place in any other universe than the Doctor's.
